LIABILITY FOR DAYCARE/BOARDING/WALKING I understand that any and all bites caused by my dog towards staff, other clients, or dogs during the time my dog is in the care of BowWow Lounge and their employees, will be reported to the local authorities as is required by law.
I understand that I am liable for any medical care expenses and damages that result from injuries caused by my dog.
I expressively understand and agree that the BowWow Lounge shall not be held responsible for any damage to property or that of others caused by my dog during the period in which they are in its care.
I expressively waive and relinquish any and all claims against the BowWow Lounge, its employees and representatives for daycare, boarding, walking and transportation purposes.
I have disclosed to BowWow Lounge all known dangers associated with my dog.
I understand that under no circumstances will the BowWow Lounge be liable for consequential damages to or caused by my dog.
If any medical problems arise while my dog is in the care of the BowWow Lounge, I authorize the BowWow Lounge to do whatever they deem necessary for the safety, health and well being of my dog. Further, I agree to assume full financial responsibility for any and all expenses incurred.
I understand that the BowWow Lounge is a cage-free daycare facility. I accept the risks involved and agree that the BowWow Lounge is not liable for any injuries or illnesses resulting during my dog's attendance.
I understand that cuts, abrasions and other unknowns may arise while my dog is in daycare, boarding or out on a walk. I accept that the BowWow Lounge reserves the right to discuss the details of any such instance if and when deemed necessary.
I understand that BowWow Lounge is a cage facility during the day and for overnight purposes and that the dogs are monitored by remote system from 8pm-6am each day.
If I authorize my dog leave the premises during daycare or boarding hours, I release BowWow Lounge, their employees and representatives during that time.
I understand that Boarding dogs are enrolled into the daycare facility during each of the days my dog boards. Unless I state by written submission, my dog will be enrolled and socializing during certain hours each day they board with BowWow Lounge. I will also adhere to daycare rules and regulations.
I understand that the BowWow Lounge has the right to refuse service to me and my dog at any time and for any reason.
I hereby declare to the BowWow Lounge that I am legal owner of my dog; that my dog has not been exposed to distemper, rabies, parvo or bordatella within the past (30) thirty days; that my dog has been inoculated as indicated by records presented; that my dog is currently and properly licensed through the city; and that I (the owner) have read this agreement in its entirety.
I understand that by checking all above boxes that this document is considered legal and binding.
PAYMENT FOR DAYCARE/BOARDING/WALKING I understand that all package rates are non-refundable. Period.
I understand that payments and packages for boarding and daycare are handled upfront and never after a service has already been provided by BowWow Lounge.
I understand that in order to reserve my dog's place for boarding I must have a current and valid credit card on file.
I understand that the daycare hours of operation are 7:00am-7:00pm Monday-Friday / 10am-5pm on Saturdays and a $10 late fee charge applies per 15 minute increment until 7:30pm / 5:30pm. If I have not picked my dog up by 7:30pm / 5:30pm, the BowWow Lounge reserves the right to board my dog overnight in any available private suite, deluxe suite or mini-suite, whichever is available. at my expense. I authorize, in advance, that should this ever happen my credit card will be charged.
If I purchase 1/2 day service and fail to pick my dog within 4 hours I will be charged the full day rate of $28. If I fail to pick up my dog by 7:00pm / 5:30pm, additional late fees will apply (see above).
I understand that if I have reserved boarding for my dog(s) and do not cancel within 24hrs of my start date, I will be charged one night at the rate I had arranged for. If it is a holiday booking and I fail to cancel within 72hrs of my start date, I will be charged 2 nights at the rate I had arranged for.
I authorize the BowWow Lounge to charge my credit card for any outstanding balance on my account, plus a $5 fee for processing.
I understand that if an unpaid balance remains unpaid for 60 days or more, unless another arrangement is made, my BowWow Lounge account will be turned over to a collection agency and that as a result I will be liable for collection costs and attorney fees in addition to the unpaid balance.
I understand that the BowWow Lounge will charge me a $35 handling fee for all returned checks.
